Estrace vaginal cream is contraindicated in patients with:
- Active DVT or PE, or history of these conditions [1]

For genitourinary syndrome of menopause symptoms that persist despite nonhormonal measures, low-dose vaginal estrogen is recommended in appropriate patients (this recommendation does not override the product’s contraindication for prior VTE). [2]
Common Clinical Approach When Prior VTE Exists
- Nonhormonal vaginal moisturizers and lubricants are used first when vaginal atrophy symptoms occur in patients with contraindications to vaginal estrogen. [2]
- Specialist (gynecology and/or the clinician managing anticoagulation) input is indicated before any estrogen-containing therapy given the Estrace contraindication for prior DVT/PE. [1]
