Vitamin K (Phytonadione) Dosing for Pediatric Acute Hepatitis With Elevated INR

Vitamin K repletion is recommended in pediatric acute liver dysfunction with coagulopathy based on an age- and weight-appropriate dose range. [1]

  • Infants: 1 mg vitamin K (phytonadione) [1]
  • Older adolescents: up to 10 mg vitamin K (phytonadione) [1]

Route of Administration

  • Oral vitamin K therapy should not be used to correct the INR in pediatric acute liver failure. [1]
  • Subcutaneous administration should be used to ensure administration. [1]

INR-Guided Escalation

Dose and route should be reassessed based on response of coagulation indices. [1]
