Olanzapine Starting Dose for Adults

For adults with schizophrenia, oral olanzapine is recommended to be started at 5–10 mg once daily, with a target of 10 mg/day within several days (Class I recommendation for indicated dosing in the U.S. prescribing information). [1]

For adults with bipolar I disorder (manic or mixed episodes), oral olanzapine is recommended to be started at 10 or 15 mg once daily. [2]

Lower Starting Dose in Sensitive or Debilitated Patients

In patients who are debilitated or who have a predisposition to hypotensive reactions or slower metabolism/pharmacodynamic sensitivity (examples include nonsmoking female patients ≥65 years of age), the recommended starting dose is 5 mg. [1][2]
