Vaginal crawling sensation differential
A crawling sensation in the vagina is a form of abnormal sensation (paresthesia) that can occur with neuropathic vulvovaginal pain disorders, but non-neuropathic gynecologic causes must be excluded first. [1][2]
Neuropathic vulvovaginal pain disorders
Vulvodynia (chronic vulvar pain lasting ≥3 months without a clear cause) can be associated with abnormal sensations described as burning, stinging, irritation, and rawness around the vaginal opening. [1][3]
Vulvodynia evaluation includes ruling out treatable causes such as active yeast or bacterial infection, sexually transmitted diseases such as herpes, and inflammatory skin conditions. [2]
Pudendal neuralgia (pudendal nerve irritation or entrapment)
Pudendal neuralgia is chronic pelvic and perineal neuropathic pain caused by injury, inflammation, or irritation of the pudendal nerve. [4]
This condition can cause burning and tingling (nerve pain) with genital involvement that may include the vagina. [5][4]
Infectious and sexually transmitted causes
Vaginal or vulvovaginal pain with abnormal sensations can be caused by infections, including yeast infection and bacterial infections. [2]
Vaginal or vulvovaginal pain with abnormal sensations can also be caused by sexually transmitted infections, including herpes. [2]
Inflammatory and dermatologic vulvar disorders
Inflammatory skin conditions can cause vulvar pain syndromes that may be perceived as abnormal sensations. [2]
Dermatologic causes listed in clinical references for vulvar pain include lichen sclerosus, lichen planus, and contact dermatitis. [3]
Irritant or contact-related vulvar inflammation
Contact dermatitis can produce vulvar pain, which may be described as irritation and burning-type sensations and may be associated with genital “unusual sensation” symptoms in vulvovaginal pain evaluations. [3][2]
Related diagnostic principle
Chronic vulvar pain syndromes without an identifiable cause (such as vulvodynia) are diagnoses of exclusion and require evaluation to identify infections and dermatologic/inflammatory causes. [1][2][3]
Common clinical triage indicators for urgent evaluation
Immediate medical evaluation is warranted when genital symptoms are accompanied by systemic or severe features, such as severe pain, rapidly progressive symptoms, or symptoms suggestive of herpes infection. [2]
